What is Mobile Integrated Health?

MIH is a progressive model of community-based care that sends EMS providers—working in a non-emergent capacity—to deliver assessments, education, and connections to the right resources. Care is tailored to each person’s needs and delivered in clinical and non-clinical settings.

Paramedics provide medical assessments, preventive health education, and help individuals connect with social services—acting proactively to close care gaps.

The Problem We’re Solving

Hardin County faces a shortage of mid-level providers and limited fast-track access for non-urgent needs. Post-discharge follow-up is challenging, which can drive ER and 911 overuse and readmissions.

Program Goals

No. MIH is for non-emergent needs. If you are experiencing an emergency, call 911.

Does MIH replace my doctor?

No. We complement your existing care and help you follow your provider’s plan.

Is there a cost?

Costs, if any, depend on services but most do not have a cost to the patient. Call us to discuss options.

Can family or caregivers request a visit?

Yes. Caregivers can call or submit a referral with patient consent. We will verify consent before scheduling.
